Homecare CoordinatorFormal position title. Reports ToCoordination Manager (US) and Team Leader (BAQ).Job OverviewCoordinate resources to deliver service to patients.

Keep a two-way communication with insurance companies and other organizations on changes in the clients' Plan of Care.Responsibilities and Duties

  • Manage caseload by providing care coordination to individuals with a disability diagnosis or special needs
  • Verify daily duty logs for completion and accuracy according to HIPAA standards
  • Maintain monthly home care reports and update client service records as needed in CRM.
  • Act as a liaison in planning home hospice and provide emotional support to patients and their family members.
  • Determine caregiver availability and schedule staff appropriately.Qualifications

  • Attention to detail.
  • Ability to communicate and work effectively with patients, co-workers, and partners.
  • Communication skills in both, oral and written form.
  • Display ability to work in a multi-tasking working environment.
