Manage caseload by providing care coordination to individuals with a disability diagnosis or special needs
Verify daily duty logs for completion and accuracy according to HIPAA standards
Maintain monthly home care reports and update client service records as needed in CRM.
Act as a liaison in planning home hospice and provide emotional support to patients and their family members.
Determine caregiver availability and schedule staff appropriately.Qualifications
